The raw data output from your run does not appear to have a year 
associated with the dates.  E.g., here is what came out when I ran 
your input file with the weather data:

So, I'm thinking that our "month/day" convention is causing you some 
problems with your regional date convention or opening in Excel or 
something similar.    (After you open the file in a spreadsheet, it 
has automatically read some things as dates and performed its own conversion).

EnergyPlus does read the regional date settings for some outputs 
(e.g. the regular eso and meter files as I remember) but looks like 
this may need to be applied for the Map output as well.

I will research a little more and write up a Change Request if that 
appears to be the problem.

Sorry for the inconvenience.  A work around for you might be to try 
setting the regional date settings to month/day convention rather 
than what you are now but I'm not sure that will work.
