1a.  In EnergyPlus, the only way to introduce outside air (ventilation air) in 
an HVAC system is to use a mixer, even if the relief air flow rate will always 
be zero.

1b. Yes, it is always recommend that you combine into one system whenever 
possible if the model will be equivalent.

2.  This depends on how the system will be controlled.  If the supply air 
temperature from the ventilation system is fixed, then use DIRECT AIR.  If each 
zone has independent control of its supply air temperature, then use SINGLE 
DUCT:CONST VOLUME:REHEAT.

3.  Yes, it is recommended that you do so.
