UFAD has been a part of EnergyPlus for quite a
while.

If you have installed EnergyPlus look for 
RoomAirSettings:UnderFloorAirDistributionInterior
and
RoomAirSettings:UnderFloorAirDistributionExterior 

in the Example Files spreadsheet for appropriate input files.
