Koji,
I renamed your file as TokyoAMeDAS.dat to match the name of the DEF file (TokyoAMeDAS.def), and ran it without any difficult. I've put the output TokyoAMeDAS.epw on my company website for download:

I don't know what was causing your problem, but here were the steps I used:
1. make sure the data file has the same name as the DEF file
Then, in the Weather program,
2.click Select File to Convert and select the data file
3.click Override Default Type, select the first option "Custom Data Format (requires DEF File)", and hit OK. This step is a little confusing because nothing comes up until
you click the down arrow on the right. Perhaps you failed to do this step?
4. click Select Output Format (automatically shows EPW)
5. click Save File As and then hit either OK or change the file name. I generally just tell it to use
the same file name, but with the EPW extension.
That's it.
