This thread has taken some turns and I'm not sure if everything has been 
answered - sorry if I'm duplicating.

1.  Answered elsewhere by Linda.

2. EnergyPlus only models the case of a heat exchanger embedded in the pond..  
Increasing the quantity of tubing and decreasing its thermal resistance (thin 
with high conductivity) should approach the direct fluid case.

3.1 Yes.  I assume you've looked at the pond example file?  
PondHeatExchanger.idf

3.2 Yes.

4. Not exactly.  The pond will not be simulated if it is not connected to a 
plant or condenser loop, so it must be part of a larger model.  I have not 
tried this, but I think you could start with the example file 
PlantLoadProfile.idf, add the pond, schedule all of the pumps off, and see the 
pond temperatures.
