I think the confusion is in the prompt message, which is not the EPlus original.
One can run simulation for sizing periods using designDay objects with weather file set to "none".    The designDay objects in the .ddy files are not used for simulation.  There are also some "specialDays" defined in some .epw files.

These are from the .idd file.
     
Using  "Yes,!- Do the design day calculation"  as  A4 field prompt line  implies that only design day object is used for that field, and sizing is not mentioned. The difference between the weather file run and sizing run is now not clear.
 
I realize this could be a problem for the third party programer to correct.

No, it is not correct.

It will use the SizingPeriod:* objects to determine sizing but will not "run" them (and produce simulation results).  There are SizingPeriod objects that will select days from the weather file.

   Yes, !- Do the design day calculation

If you choose this, then it will "run" the SizingPeriod:* objects in your input file and produce simulation results.
