The true definition would be per surface and is as described in e+ documentation. However, ASHRAE 90.1 dictates the factor should be read of a table (regardless of perimeter) and is thereby assuming a 150mm slab with xmm insulation...I.e. becomes a factor of insulation only. So you work backwards: get the required F-factor from table 5.x, lookup the corresponding F-Factor in Appendix A to find the required insulation R-value for your 150mm slab.
